Received Team Applications

Registration is open to any properly affiliated Girls U13, U14, U19 and Boys U13-U19 teams that are registered with a WI Youth Soccer Association affiliated club in “Good Standing”. Group Play for U13-U14 Girls and U13-U15 Boys will take place at USA Park in Appleton, U16-U17 Boys will take place at Score Park in Racine, all over Memorial Day Weekend. Semi-finals and Finals for U13-U17 Boys and U13-U14 Girls will be May 31, 2008 and June 1, 2008. Group Play for U18 Boys will be held at Fond Du Lac Soccer Complex on May 10, 11, 17, with Semi-finals on May 18 and the final will be May 21, 2008. U19 Boys and Girls will be determined between the teams that apply at a later date. Teams drawn into a play-in game and not advancing to the group play round robin competition will have a portion of the entry fee returned to the listed team contact at the conclusion of the Spring US Youth Soccer Wisconsin State Championships series finals.

Bid to Host Fall 2008 and Fall 2009 State Championships
This is the bid to Host the Fall 2008-2009 State Championships, which is for the U15-U18 Girls teams. Please also note that this is a 2 year bid so if your bid is accepted you will be required to host both Fall portions of the State Championships for 2008 and 2009. The dates for Fall 2008 State Championships are Sept. 13, 14 and 20th with the Semifinals on Sept 21, 2008 and the dates for Fall 2009 State Championships are Sept. 12, 13 and 19th with the Semifinals on Sept. 20th, 2009.  The bid document is due to Carl and Bryan Flanagan on March 4th, 2008, please send document to both cmicke@gmail.com and bflanagan@wiyouthsoccer.com. 2008 Required State Championships Forms - Please Read
The 2008 Wisconsin State Championships Rules are available for your review. The State Championships Committee made several changes. Please make your players familiar with the changes.

The forms required for participation have also been changed. The “BLUE ROSTER” and official State stamped team roster is to be brought to registration at State Championships before your FIRST game. Also be sure to bring along those items in the email from Carl Micke to all State Championships Managers. This form must be signed by all players who expect to participate in the 2008 event. Please include a copy of each roster that shows the name of any player used under the “club pass” provision

The team manager and coach will be expected to sign and complete the following forms; Team Information, Eligibility form, Code of Conduct and the Game Day Roster.

These forms must be presented by the team representative at the designated “team check in” area at the tournament venue 30 minutes prior to the first scheduled game.

Teams that are scheduled to participate in a play in game must bring copies of the four forms to the Play In Game, and give to the Wisconsin Youth Soccer Official and if there is not one, give to the referees. The play in game victor must then present the original form to the “team check in” area at the tournament venue. The game day roster and player passes must be presented to the match official prior to all play in games. Remember the payment to the referee must be made before the start of the play in game. In addition the manager or coach must have in their possession the player medical consent forms and Waver of Liability at all times.

State Championships Email List
We have created an email subscription list for the State Championships. All communications regarding the tournament will be sent by email only. All team managers are required to Subscribe to the State Championships Email List. Others wishing to receive email updates on the State Championships tournament are invited to subscribe to the State Championships email subscription list as well.
